{"tool":"get_job","result":{"requisitionId":"989632","client":"Parkland","title":"Supervisor Social Work Behavior Health Services","description":"Location: Main Hospital Bldg - 1st Flr<br><br>Primary Purpose<br><br>Responsible for the coordination and management of daily operations of assigned Social Work area, including serving as a resource and role model for clinical practice, training, assisting social work leadership, evaluation and supervision of practitioners and students in all areas encompassed by the service line(s) to ensure optimal quality of patient care. Responsible for performing psychosocial screenings and assessments and implementing the psychosocial plan of care for assigned area for all patients to ensure the patient has sufficient resources and support services.<br><br>Minimum Specifications<br><br>Education<br><br>&bull; Must have a Master's Degree in Social Work.<br><br>Experience<br><br>&bull; Must have six (6) years of Social Work experience.<br><br>&bull; Prefer previous health care based management experience.<br><br>Equivalent Education and/or Experience<br><br>Certification/Registration/Licensure<br><br>&bull; Must be licensed by the Texas State Board of Social Work.<br><br>&bull; Must have Licensed Master Social Worker (LMSW).<br> &bull; Prefer Licensed Clinical Social Worker (LCSW).<br><br>Required Tests for Placement<br><br>Skills or Special Abilities<br><br>&bull; Must have excellent verbal, written and organizational skills.<br><br>&bull; Must have grant development and management skills, and be detail oriented.<br> &bull; Must have excellent interpersonal skills and ability to communicate effectively with a diverse population of patients, staff and community agencies.<br> &bull; Must demonstrate knowledge of community resources and services of benefit to targeted population.<br> &bull; Must demonstrate patient centered/patient valued behaviors.<br> &bull; May be required to be fluent in reading, writing, and speaking the Spanish language.<br><br>Responsibilities<br><br>Works with Social Work Management. Assists in selecting, training, scheduling, motivating, supervising, evaluating and makes recommendations for disciplinary actions up to and including termination, to ensure maximum utilization of individual and group capabilities. Ensures that assigned employees receive opportunities to further their knowledge.<br><br>Collaborates with management team in service areas to develop and implement a system for providing social work coverage and accountability that meets the needs of patient care and organizational efficiency.<br><br>Develops, implements and evaluates a formal, structured orientation and continuing education program for the social work staff to ensure competency of staff, promote progressive learning and development of expertise, support practices consistent with advancements, and promotes compliance with internal policies, procedures, and regulations. Maintains documentation of educational activities provided. Revises education programs based on identified evaluation criteria and feedback. Develops and implements action plans as indicated when knowledge and skill deficits are identified among staff. Some programs may include<br><br>Complex Care Committee or Inpatient HBIPS CMS requirements.<br><br>Performs psychosocial assessments that may include diagnosis on the five axes in DSM-V to ensure all patients needs are properly identified and appropriate interventions are integrated into the plan of care. Assesses patient's and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope, understanding of present circumstances and patient/family priorities/needs. Collaborates with the patient, family and multidisciplinary team to assure psychosocial needs are addressed. Utilizes review of medical record, interview of patient/family and consultation with the interdisciplinary health care team to complete screenings/assessments/plan of care.<br><br>Counsels patients and families regarding emotional, social, and financial consequences of illness and/or disability and other psychosocial issues identified.<br><br>Provides crisis intervention and counseling for patients and families. Promotes patient and family empowerment and independence.<br><br>Participates in discharge planning activities. Implements psychosocial portion of plan to facilitate timely patient discharge and promote optimal utilization of patient and community resources. Provides planning for a safe discharge.<br><br>Accesses and mobilizes family/community resources to meet identified needs. Intervenes with families with complex psychosocial needs and family dynamics.<br><br>Educates patient/family and physician about post-acute options and addresses issues of choice.<br><br>Validates discharge criteria/plans for patients and families and notifies the interdisciplinary team of newly identified resources or changes in previously identified plans/resources.<br>Provides intervention in cases involving child or adult abuse/neglect (serves as liaison between Parkland and Child Protective Services/Adult Protective Services), domestic violence, guardianship (temporary/ permanent), foster care, adoption, mental health level II placement, and sexual assault. Initiates Child Protective Services/Adult Protective Services contact and mandatory reporting.<br>Documents screening, assessment, interventions and outcomes to ensure effective communication per established documentation procedures.<br><br>Job Accountabilities<br><br>Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.<br><br>Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.<br><br>Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans.
